Answer: #1 water, like all fascia, is highly important for the quality of the fat pad, micro chamber and macro chamber constructs #2 the venous hydraulics of the lower leg and foot. If this mechanism is less than efficient, the forces increase to the calcaneus, fat pad and changes the trabecular forces of the foot and lower limb!

Understanding the “fascia” includes orthopedic fat pads, is a fundamental part of both differential diagnosis and specific treatment.
Hoffa’s fat pads must remain “fluidic” in nature, especially in post surgical knee recovery. I will be addressing many facets of the “fascia” of the lower limb in December at the Complexity of the Ankle and Foot course. REMEMBER…fascia is the home of all important Proprioceptors! Quality matters.

Dr. Amstutz went to the Return to Play conference this past weekend and wanted to share some key insights that he left the weekend with.

▪️ Alan Murdoch spoke about the differences and limitations of the ‘coach’s eye’ skill/phenomenon. He did an incredible job with the audience on a couple of ACL Reconstruction cases as they were back on the field moving…He works with a speed first paradigm of rehabbing ACL and presented some incredible data on the difference in forces between running slowly vs fast running.

▪️ David Power presented on a clinical case involving a distally torn SemiTendonosis with 7cm of retraction. The rehab and surgical opinions decided on rehab and no surgery. The pro athlete is back on the field after a rigorous program of manual therapy, corrective exercise and progressive S&C. Data was taken weekly on many parameters and the 11 week journey was an arduous one.

▪️ Bill Knowles the OG ReConditioning Expert started the day with his cutting-edge approach to injury and post-surgical management. ‘Every orthopedic insult comes with a neurophysiological one…don’t wait to re-establish the proper movement patterns. “Words create images, Images Actions”. Before you dive into the fascia, ask: is there space in the joint?

Too often, we go straight to the layers — the ligaments, tendons, and connective tissues — without first checking if the joint itself even has room to move.

Here’s the thing:
➡️ A compressed joint = limited fluid flow
➡️ Limited flow = stuck fascia
➡️ Stuck fascia = compensation, tightness, pain

💡 Creating space and fluidity at the joint is step one.
