PART TWO
73 year old male presents with low back pain and difficulty walking. He is now post op from his neck surgery almost a year with interval improvement of his low back pain presumably from better neck alignment. He complains of worsening ability to walk distances with need for frequent breaks. He decided to undergo an anterior and posterior lumbar reconstruction. Postoperatively, he is very pleased with outcome with significant improvement of his back and leg pain and numbness with improved ability to walk distances.

PART ONE
73 year old male presents with low back pain and difficulty walking. On physical examination, patient has reflexes concerning for spinal cord injury. MRI of the cervical spine shows 3 levels of cord compression (see yellow arrows). Preop x ray showed kyphotic deformity. Patient underwent 3 level anterior reconstruction with postop improvement of his back pain and ability to walk. Back pain improvement may be due to the better position of the head. 50 year old female fell from a 6 fr ladder while decorating. She suffered an uncommon fracture involving the superior articulating facets of L5 on both sides resulting in severe back pain and nerve pain down the legs. Initially she did not want to undergo surgical intervention and elected for a brace only. Subsequent X-rays show worsening of the malaligment between L4 and L5 (blue line) and she experienced worsening nerve pain. Minimally invasive transforminal lumberinterbody fusion was carried out with resolution of her back pain and nerve pain.

33 year old right handed male complains of neck pain radiating into the right arm with weakness in the right arm. MRI showed disc herniations at C5/6 and C6/7 with narrowing around the spinal cord and nerve roots. Nerve studies showed nerve damage at the right C6 and C7 nerves. He underwent decompression of the nerves and spinal canal with 2 level artificial disc replacement. Follow up X-rays show motion preservation at the artificial discs. Postoperatively, patient is pain free with resolution of his arm symptoms.

62 year old female presents with progressively worsening back pain and difficulty walking. Patient had a previous history of endometrial cancer. While undergoing oncological work-up, patient suffered neurological deterioration to becoming wheel chair bound and unable to lay down. Contrast enhanced MRI showed L2 vertebral metastatic lesion extending into the epidural space compressing the nerves and into the left psoas and retroperitoneal space (green highlight). Whole body PET CT scan showed a single spinal lesion (bright yellow-orange). Patient underwent an all posterior resection of tumor and reconstruction of the thoracic and lumbar spine (see xrays). At six weeks post op, she was walking a mile daily with excellent recovery of neurological function and resolution of 90% (self reported) of her preoperative pain. Patient underwent adjuvant oncological therapies postoperatively with 3 month post operative PET CT demonstrating no evidence of hypermetabolic malignancy.

A local to Southern California, Dr. Yanni attended Calabasas High School before graduating summa cm laude from UCLA in Neuroscience. He then received his medical degree from the University of California, San Diego. He completed his internship in general surgery at Cedars-Sinai Medical Center in Los Angeles, followed by a residency in neurological surgery at Rutgers / New Jersey Medical School. Subsequently, he completed his fellowship in minimally invasive, thoracoscopic, and complex spine surgery at St. Luke’s Roosevelt Hospital in NYC. He was then Asst. Professor and Director of the Comprehensive Spine Neurosurgery Service at UC Irvine before starting Disc Comfort, Inc.

Dr. Yanni has extensively published his clinical expertise and research in more than 30 peer reviewed journal papers, book chapters, and reviews.

In addition to standard spine surgery, Dr. Yanni performs spine surgery through key-hole size incisions, a minimally invasive procedure that allows patients to be released within hours or days and return to work within weeks. Traditional open surgery typically involves longer recovery and rehabilitation times.
