If you’d like to see whether you qualify, please email us at

nicholasville@medsaverx.com with:

Your completed screening form

A photo of your medical insurance card (front and back)

Our team will review your information, confirm your eligibility, and

submit your request to Exact Sciences, the manufacturer of Cologuard.

They will verify your insurance coverage and mail the kit to your home

in a discreet, easy-to-use package.

Eligibility includes:

• Adults ages 45–85

• Individuals at average risk for colorectal cancer

• Willingness to complete a colonoscopy if the result is positive

If you’re experiencing symptoms or have a history of polyps, colorectal

cancer, IBD, or a significant family history, we may recommend a different

screening approach.
