For LVAD patients, the journey doesn’t stop once they leave the hospital- it simply shifts to managing care from home.

Remote monitoring helps clinicians stay connected to important health data between visits. Tracking INR levels, vital signs, and other key indicators allows care teams to stay informed and respond when adjustments are needed.

When patients and providers have the right information at the right time, it helps create confidence and continuity in care.

At ODI, supporting this connection between patients and care teams is an important part of helping individuals manage life at home.

For LVAD patients, the transition from hospital to home is an important milestone- but it often comes with new routines and responsibilities.
From monitoring health data to managing supplies and learning daily care practices, the first weeks at home can require guidance and support for both patients and caregivers.

With the right resources and coordination in place, this transition can become a confident step forward rather than an overwhelming change.

At ODI, we’re proud to support patients and care teams throughout this important stage of the journey.

What if managing LVAD logistics felt less fragmented?

VADTrac gives care teams a single, connected view of the information they rely on every day- from INR results and vital signs to equipment tracking, supply oversight, and recall notifications.
Instead of juggling multiple systems, clinicians can monitor patients, review data, and manage inventory from one secure dashboard.

It’s not just about data. It’s about having the right information, at the right time, in one place- so care stays proactive, not reactive.
