What makes Rosewell different is our clinical oversight. Our program is led by a Dual Board-Certified Nurse Practitioner in Family and Psychiatric Mental Health Nursing with neurology experience focused on recognizing early behavioral and cognitive changes in individuals living with mild to early moderate dementia and chronic neurological conditions.

• Mood & behavior changes
• Increased confusion
• Functional decline
• Sleep/appetite changes
• Safety concerns

We collaborate with healthcare providers to help identify concerns early and support timely interventions when changes occur.

❤️ Every participant receives a Person-Centered Service Plan developed around their goals, preferences, health needs, strengths, and independence.

They may can not tell you what they like or dislike but knowing a persons dislike and likes from family helps us to develop a person centered care plan for each participant.

This participant is a well known prosecutor and guess what we are watching, Law and Order. He has a smile on his face and is engaging with it. This was after a morning walk and sitting and talking about his career as a prosecutor (I looked him up and asked him questions about what I found). This is reminiscence therapy and his a part of his person centered plan.

Our participant will never adjust to us, we adjust to them. 💙
