Helping older adults stay safe, independent, and at home is at the heart of what we do.

Our Day Kimball Health at Home team recently hosted a Fall Prevention Clinic in Plainfield at Marie’s Fitness & Massage Center, where participants received fall risk screenings and practical tips to improve balance and reduce fall risk.

This clinic is part of our ongoing Fall Prevention Program, supported by Senior Resources – Agency on Aging through Title III funds made available under the Older Americans Act.

Thank you to Marie’s Fitness & Massage Center for hosting and to everyone who joined us in taking steps toward safer, healthier living.

We could not be more proud of these two home health and hospice clinicians!

We’re proud to recognize two exceptional clinicians as this year’s Day Kimball Homecare Homecare Hero Award recipients.

Named in honor of a longtime home health nursing leader, the Dianne Dupere-Lindell Award for Excellence in Home Health Care recognizes exceptional clinical excellence and leadership. This year's winner, Shawn Smith, RN, CRNI, has been a registered nurse for 31 years, with a strong background in acute dialysis, oncology, and IV home infusion therapy. Since joining DKH at Home in 2023, she has made a significant impact as the team's IV resource nurse. She serves as a primary preceptor and mentor, supporting and strengthening our nursing team through her expertise, calm leadership, and commitment to excellence.

This year's Ann Edwards Award for Excellence in Hospice Care, named for a beloved hospice nurse and former DKH Employee of the Year, was presented to Cindy Chapman, RN. Cindy has been a registered nurse for 14 years and has experience in hospice, assisted living, and home health. She joined DKH at Home in 2019. Known for her positivity, compassion, and strong clinical skills, she is nearing completion of her BSN in Holistic Nursing, reflecting her commitment to growth and her belief in the healing art of nursing.

Congratulations, Shawn and Cindy! Together, you exemplify the clinical excellence, mentorship, and compassion that reflect the spirit and legacy of both Dianne and Ann.

Our beautiful Celebration of Life Candlelight Ceremony was held at Cornerstone Baptist Church in Danielson, where we gathered with the families and friends of those cared for by our hospice program over the past year. It was a heartfelt evening of remembrance, reflection, and community. We extend our deepest gratitude to Reverend Yana Cruz-Pagan, the Killingly High School Concert Choir and their director Jason Largent, Chubby Dog Coffee Co., Lapsley Orchards, Putnam Stop & Shop, and our incredible Hospice Volunteers and staff for helping make this special night possible.

We were honored to meet with Congressman Joe Courtney, the Connecticut Association for Healthcare at Home, and Aveanna Healthcare to discuss proposed Medicare cuts that could reduce home health payments by over 9%, with even deeper impacts in rural communities like ours. These cuts would directly affect the care patients rely on at home. We’re grateful for Congressman Courtney’s advocacy—and we encourage everyone to raise their voice to ensure home health care remains accessible for all.
