Reducing Readmissions and Supporting Safe Recovery Through Post-Hospital Care in Palm Beach County

Hospital discharge is meant to signal improvement. A senior has stabilized, treatment is complete, and they are cleared to return home. But for many older adults, the days and weeks following discharge are when the greatest risks begin.
