Are you passionate about making a difference in the lives of individuals with intellectual and developmental disabilities or Autism? Journey to Wellness Supports Coordination (JTW) is seeking dedicated and compassionate Supports Coordinators (SCs) to join our team!
This is a hybrid position with flexible scheduling.

Journey to Wellness Supports Coordination (JTW) – North Versailles, PA.
Pay: $25/hour | Flexible Schedule/Up to 30 hours a week
Will be paid $19 (base pay) until you are trained for your entire caseload.

Compensation $25/hour is based on billable interactions and activities with your assigned clients.

CELL PHONE AND LAPTOP PROVIDED.

About the Role:
Supports Coordinators are responsible for locating, coordinating, and monitoring services for both children and adults with intellectual disabilities and/or Autism.

Key Responsibilities:
Coordinate and monitor support services for individuals with IDD and Autism
Serve as a primary point of contact for individuals and families
Maintain accurate documentation in accordance with state and agency standards
Ensure individuals receive appropriate care and community resources
Work independently while collaborating with the team as needed
Maintain Budget for clients with waiver (Basic math skills needed).

Requirements:
Bachelor’s degree in Social Work, Psychology, Sociology, or a related social science
(Must include at least 12 college credits in an eligible field)*
OR
Associate degree (or two years of college with 12 eligible credits) AND two years of paraprofessional case management experience
Strong interpersonal and communication skills
Organizational skills and time-management abilities
Proficiency in Microsoft Word
Ability to work both independently and collaboratively
Valid driver’s license and access to a reliable vehicle

Preferred Qualifications:
Experience in case management or a related healthcare/social services role
Customer service experience in a human services environment
