We are committed to providing skilled, compassionate care to patients in the comfort of their own homes. We are growing and currently hiring LPNs, RNs, and Physical Therapists to join our professional care team.

We Are Hiring:
Licensed Practical Nurse (LPN)

Registered Nurse (RN)

Physical Therapist (PT)

Responsibilities:
Provide in-home skilled nursing or therapy visits based on the physician’s orders

Administer medications, treatments, and wound care as appropriate to licensure

Perform physical assessments and evaluations

Develop and implement individualized care or therapy plans

Educate patients and caregivers on clinical care and best practices

Maintain accurate, timely clinical documentation using EMR

Communicate and coordinate care with physicians and the interdisciplinary team

Qualifications:
LPN / RN Candidates:

Active Illinois nursing license (LPN or RN)

Current CPR certification

Minimum 1 year of nursing experience (home health experience a plus)

Reliable transportation and a valid driver's license

PT Candidates:

Active Illinois Physical Therapist license

Current CPR certification

Minimum 1 year of PT experience (home health experience preferred)

Ability to create and follow therapy plans tailored to each patient

We were honored to participate in the Buffalo Grove 2025 Senior Expo at the Community Arts Center. This event, hosted by the Village of Buffalo Grove and the Buffalo Grove Park District, provided a valuable opportunity for seniors and their families to connect with a variety of local service providers. ​

At Homestead Health Care Services, we were proud to showcase our comprehensive offerings, including Home Health, Home Care, and Wheelchair-Accessible Transportation services.
