We are excited to share with you one of our recent rehabilitation success stories! Meet Ms. Alyssa Jacks who worked with our awesome therapy team at Deerfield Crossing of Northbrook towards increased independence and functional goals. We are proud to share her story and her success!
Ms. Alyssa Jacks transferred to Deerfield Crossing of Northbrook in April, 2023 from another skilled nursing facility following hospitalization for Covid-19, sepsis, and an abscess which required surgery. Prior to her hospitalization, Alyssa was independent within the community while using a four wheeled walker and was able to drive. Upon her arrival to our facility, Alyssa was extremely weak and deconditioned leaving her bed bound. She reported 7 out of 10 on the pain scale to her left leg with exertion. Initially, Ms. Jacks was a two person total assist for transfers in/out of bed and lower body dressing. In addition to her initial issues, it is worth noting that Alyssa’s rehab journey was further complicated by the development of a blood clot to her leg that contributed to her immobility. Currently, Alyssa has improved to the point where she is able to complete lower body dressing with supervision and she is able to complete upper body dressing when items are set up for her. Alyssa has been working extremely hard on her ability to complete bed to wheelchair transfers with set-up assistance and is able to walk 30 feet with minimum assistance with a front wheeled walker. Despite the many obstacles along her path to recovery, Alyssa’s strong determination to improve, along with the knowledge and expertise of the staff at Deerfield Crossing of Northbrook, has given her the opportunity to continue making tremendous strides towards achieving her goals. Alyssa will continue progressing towards achieving her ultimate goal at our facility and we couldn’t be more proud of all of the progress she has made!!
