Heart disease is one of the most common reasons patients receive hospice care. For those living with advanced heart conditions, hospice focuses on comfort, symptom relief, and quality of life—wherever home may be.

If frequent hospitalizations, worsening shortness of breath, or fatigue are impacting daily life, hospice support may be appropriate sooner than many realize.

We want to highlight our third employee who we are proud to share is a published author! Jennifer Zelen DNP, RNC, and CHPN runs our Palliative Care Program.

We like to look at Palliative Care as a step before hospice. With Palliative Care, you can continue to receive treatments, therapies, etc. while also having a monthly visit with a Palliative Care nurse, just like Jennifer, on a monthly basis. This allows her to check in with you month to month, discuss your illness and what the future may look like in your specific situation, and prepare you for what is to come.

In her book, she discusses all of the above and then some! She provides a beautiful insight as to how you can delicately discuss the end-of-life plan, something that each of us will face one day.

Myth vs. Fact

Myth: Hospice is only for the last few days of life.

Fact: Many patients can benefit from months of added support when they start earlier.

Who qualifies for hospice: if you have a serious, life-limiting illness, a doctor certifies you have six months or less to live if the disease runs its normal course, and you choose comfort (palliative) care over curative treatments, focusing on quality of life instead of cure

When we hear the word hospice, most people think that the person will pass within days. Although this is the case for some, it is not true for everyone.

To be eligible for Hospice, a doctor needs to confirm the patient has 6 months or less to live. However, when someone signs up for hospice as soon as they are found eligible, studies show that 12-15% of patients live longer than the 6 months!

Those living with dementia can, in many cases, be found eligible sooner than most may think. With all types of dementia being progressive, we highly recommend reaching out to hospice as soon as you start to see any type of decline. This will allow us to screen the potential patient for eligibility. If they are not eligible yet, then we can keep checking in while also providing insight as to what to look out for and when to call again!

This week we want to recognize another Tides Hospice Employee, our Chaplain Barry, and his book A Pulse of Becoming. This book is a collection of poetry that acts as a journey to guide you to self-discovery and healing.

When dealing with loss and change, especially along the end-of-life journey, it can be hard to grasp and understand how to move forward. We hope this book sheds some light on the path forward.

We are so proud to share that THREE of our employees are published authors!

Hospice Nursing, written by one of our RN Case Managers, Peg, is a guide that covers what it means to be a hospice nurse. From the first time meeting a family upon admission, to addressing needs for the patient AND family along the way, to recognizing and planning for burnout. She covers it all. We highly recommend this book to both personal and professional caregivers!

🕊️ Winter Wellness Reminder

The colder months can be emotionally heavy. Take a moment to check in with yourself—and those you love.

Simple acts of kindness, a warm cup of tea, or quiet time together can make all the difference.

This month, we honor the compassion, strength, and love of family caregivers—those who show up every day to comfort, advocate, and care for loved ones in need. 💜

At hospice, we see firsthand the incredible dedication families bring to the journey of care. We walk alongside you, offering support, guidance, and relief—so you can focus on what matters most: cherishing each moment together.

To all caregivers: you are not alone. Your care makes a difference that words can hardly express.
