Progressive Health Primary Care DPC Model Primary Care Office. We focus on the patient and want to bring back the relationship between the doctor and the patient. We even do home visits.

We focus on preventive health but also manage all common chronic conditions. We have a satellite office for patients of the practice at 1429 N. Michigan Ave, #1429. Same phone number as our main office in Oak Park. Corner of Washington and Michigan. Across from Millenium Park. Parking available at the Grant Park North Garage.
