We have over 50 locations across the country consisting of both out-patient and in-patient facilities. Hospice services provide care and comfort to patients diagnosed with a terminal illness that has been certified by a physician. Hospice and palliative care is provided through a team-oriented approach to medical care, pain management, and emotional and spiritual support. The care-plan is developed and tailored to the patient's needs while supporting the family's wishes. Hospice focuses on caring, not curing and in most cases; care is provided in the patient's home.

Harbor Hospice is much more than a place. We are devoted to giving each family a gift when they need it most...quality of life for our patients when they are facing life's greatest challenge. OUR TEAM
Hospice care is a family-oriented approach that includes a team of doctors, nurses, social workers, counselors, and trained volunteers. They work together focusing on the patient's needs; physical, psychological, or spiritual. The goal is to help keep the patient as pain-free as possible, with loved ones nearby. Our team develops a care plan that meets each patient's individual needs for pain management and symptom control.

Our team consists of:

- The patient's personal physician;
- Hospice physician (or medical director);
- Nurses;
- Certified Nurses Aides;
- Social Workers;
- Spiritual Counselor;
- Trained volunteers; and
- Speech, Physical, and Occupational Therapists, if needed.
