Nourishing Neighbors is back and begins next week! π½π₯π
The anticipation is over; Nourishing Neighbors is back and ready to serve our communities!π π«±πΌβπ«²π»
Kicking off next week, there will be weekly free fresh produce and resource exchanges coming to a community near you! We have teamed with Garrett Growers again to bring fresh, local, nutritious produce to you. We are servicing both Garrett and Allegany Counties through the summer and early fall. The fresh produce box supply is limited and is available on a first come, first served basis. In order to receive a free fresh produce box, participants will be asked to complete a screener.
Exchanges begin at 10 a.m. and end at 1 p.m. or until produce boxes run out. We are excited for the upcoming season of Nourishing Neighbors and can't wait to see you there!ππ«π₯π₯

The Patient Centered Medical Home (PCMH) is a model for the delivery of primary health care in an office setting that is becoming widely implemented across the country as a means to build a reliable primary health care system that delivers accessible, affordable and high-quality health care.
The Patient Centered Medical home provides Comprehensive and Coordinated Care. PCMH strives to meet the majority of each patientβs physical and mental health care needs, including prevention and wellness, acute care and chronic care. This comprehensive care will be coordinated by a βteamβ of health care staff which will include physicians, advanced practice nurses and physician assistants, nurses, medical assistants, care managers, educators, social workers and the entire office staff working together to enhance the care and service within the office and community resulting in improved clear and open communication among patients and families, the medical home and the members of the broader care team.
PCMH provides for Superb Access to Care. The primary care medical home delivers accessible services with shorter waiting times for urgent needs, enhanced office hours and around the clock access by telephone to members of the health care team.
