Roots Community Health Healing our community from within through integrated health care, support services and empowerment. Roots Community Health is a nonprofit based in East Oakland.

We take an empowerment approach to all of our services and programs. We are committed to supporting each person's "whole health"--addressing not only physical and mental health, but also access to opportunity and freedom from violence. Our Mission

To provide the highest quality comprehensive health care and mental health services to Oakland residents with the goal of eliminating health disparities in our community; to identify the roots of suffering and illness and address them through community-responsive, culturally appropriate service delivery; to focus on providing excellent access to comprehensive care in a safe and dignified environment; to identify and treat not only the symptoms, but also the roots of physical and mental ailments affecting our patients and our community; to establish roots and partnerships within our community to address issues of violence, substance abuse, poor nutrition and limited access to educational and vocational opportunities; and to provide state-of-the-art, top-quality care while honoring the roots of culture, heritage and tradition.

The final day of National Negro Health Week is Reports and Follow-Up Day, where communities tracked progress, shared results, and strengthened a growing movement for Black health.

That same spirit lives on at Roots Community Health through the People’s Advisory Council and 40x40 Community Safekeeping—where East Oakland residents help shape solutions for health, safety, and wellbeing.

Because real public health is about listening, learning, and following through.

Today is General Clean-up Day, part of National Negro Health Week—a day centered on community-led cleanups and the idea that hygiene is essential to health.

That spirit lives on at Roots Community Health through service days like Martin Luther King Jr. Day and Earth Day, where volunteers come together to clean, restore, and care for shared spaces.

Because when communities care for their environment, they’re also protecting their health.

Today is School Health Day, which taught young people healthy habits through school activities, community events, and family involvement.

That same idea lives on at Roots Community Health through programs like Kick Back Friday and the Community Youth Garden—two safe spaces for hands-on learning and conversations about mental health, nutrition, and prevention.

Because when youth are given the knowledge and support to build lifelong health, they don’t just learn about health—they live it.

Today is Special Campaign Day, when communities identified urgent health issues and took action right away—holding meetings and launching solutions the same day.
That same approach lives on through Roots’ Diabetes Program, which supports people managing or at risk for Diabetes, a major health concern in our community.

“Roots has shown up for me as a place of refuge… it makes me realize that I’m not alone.”

For nearly five years, Folani has stayed committed to her recovery with the help of the care, counseling, and community she found at Roots. What began as a place to attend classes and therapy became something more: a source of consistency, encouragement, and people she now calls family.

Through Welcome Wednesday and Roots, Folani found a place that helped her stay grounded, heal, and know she didn’t have to do it alone.

Community Sanitation Day, part of National Negro Health Week.

At a time where many Black communities lived in segregated neighborhoods, poor infrastructure led to preventable illness, so this day focused on clean streets, proper waste disposal, and preventing disease through better sanitation.

Then and now, clean and safe spaces are essential to community health.

Through Roots’ STOMP program, we bring hygiene supplies, safe disposal resources, health education, and medical care directly to encampments and underserved neighborhoods—supporting healthier environments without displacing people.
