More stories about how Community Health Centers are improving maternal health outcomes: With support from NACHC partner Center for Care Innovations, the new Project 100 from West County Health Centers is creating positive changes for the health of newborn babies in their Northern California community. The project is dedicated to ensuring that every one of the 100 babies born each year in this area receives the equitable care and support they deserve.

“When I came to West County Health Centers, I wanted to continue my work with community partnerships, to focus on prevention and work ‘upstream’ to create healthy environments for babies and children in the lower Russian River area,” says West County’s Dr. Ellen Bauer.

Through innovative approaches and a commitment to equity, Project 100 is working to eliminate disparities and provide vital resources to families in need. NACHC is a proud partner of Center for Care Innovations.

Last week, the Population Health Management Initiative organized a regional learning session in Sonoma County. 🎉 Aliados Health and community health centers spent the day building a community of peers — learning key strategies, sharing promising practices, inspiring one another, identifying common challenges, and more. Our CCI colleagues were excited to join our partners in this key effort.

↗️ Collaborate with managers and senior leadership on initiatives and contribute to working groups to enhance program operations.

COMPENSATION & BENEFITS INCLUDE:

✨ 403(b) retirement plan

👉 This week (July 8-12, 2024), CCI has been taking time to slow down, reflect, and rest. Our staff will not be in office during this time.

😌 WHY DOES CCI DO THIS?

Rest is a key part of our continued recovery.

Listen to "Readiness, Response, Recovery, and Renewal," a CCI Health Pilots podcast episode recorded in 2021 at the height of the pandemic, and also the year when we first intentionally implemented this practice in the form of our summer rest / break for the entire staff.

In this episode, we discuss the 4 R’s framework which covered the different phases of the pandemic. Our hope is that this scaffolding can help health centers and other safety net organizations navigate crises as well as center rest and the 4 R's.
