Young 18-year-old patient from roughly 2 hours away, presented to our clinic with extreme lifestyle debilitating POTS, venous pooling, and pelvic congestion. After appropriate work up and trials of lifestyle modification/medications, patient underwent a procedure to investigate for compressive disorder in the abdomen. Patient was found to have some of the worst compression we have ever seen, likely resulting in her underlying POTS. Patient underwent successful repair of these vessels with two stents with restoration of normal flow, with anticipation of significant reduction of symptoms based on previous similar cases; with a satisfactory (reduction of symptoms by 50% or more) improvement success rate around 90%…

Spring is starting to show up across the Mississippi Gulf Coast!

Female patient in her mid 20s presented to our clinic with classic POTS symptoms, profound left leg swelling/pain, and currently being worked up for a hysterectomy due to unexplained debilitating pain. After medical therapy did not yield the appropriate amount of symptom relief we proceeded with a venogram and intravascular ultrasound. Patient found to have a very unique and rare bifurcating left iliac vein with near 100% obstruction and robust/significant collaterals coursing through the pelvic area dumping into the right iliac vein. Patient underwent successful minimally invasive endovascular repair. Postural Orthostatic Tachycardia Syndrome, or POTS, is not a one size fits all condition. It includes several recognized subtypes, each reflecting different patterns of autonomic nervous system dysfunction.

Some individuals experience symptoms related to impaired nerve signaling, others are affected by reduced blood volume, and some have exaggerated stress hormone responses when standing. Many patients show features of more than one subtype, which is why diagnosis and treatment require thoughtful, individualized evaluation.

Understanding how POTS presents in each person helps guide appropriate testing and care. If symptoms remain unexplained or inconsistent, specialized autonomic assessment can be an important next step.

23 year old patient presented from northern Alabama and sent over to the Mississippi Gulfcoast for care. Patient had a very symptomatic case of POTS and pelvic congestion. After failing conservative and pharmaceutical medical therapies with the inability to reduce symptoms by 50%, we moved forward with a venogram and intravascular ultrasound of the iliac venous system; also correlating with clinical presentation and physical exam. Ultimately, we found a very high grade left iliac vein compression with robust left to right collaterals dumping into the right iliofemoral venous system. After appropriate therapy and recovery, the patient now has a significant decrease of pelvic congestion and POTS symptoms; we also have been able to de-escalate her medications.
