REQUESTING RECORDS UPDATE!! Records cannot be picked up at the records facility. PLEASE DO NOT CALL THE FACILITY AS THEY CAN NOT RELEASE RECORDS TO INDIVIDUALS. All requests for records will need to come from a doctor's office. I have been in contact with the local area pediatricians and have made them aware of the process. If you are moving to an out of area doctor, please send a message and I will advise of what is needed to get those records sent. 121's (if your child is up to date on shots) are available for you to print at home on https://myirmobile.com/ just follow the instructions on the website. Please be advised older charts or those who haven't been seen in years may no longer be available. Thank you all for allowing us to be a small part of your childrens lives for all these years. We cherish all the memories that we got to be a part of and wish you all health and happiness.

REQUESTING RECORDS: for those who were unable to pick up your child(rens) records before we closed, I am in contact with the storage facility where they are now being stored. As many of you may have heard, Dr. Fain is retiring at the end of the year. He is and will continue to see patients until December 31. You are free to have your records transferred to another clinic or pick the physical records up at the office. We require a 48 hour notice for those picking up records in order to have the chart and necessary paperwork ready. Please be aware that if/when you have records transferred/picked up we will no longer be able to see you as a patient. All of us thank you for allowing us to have a small spot in your child's life and all will be greatly missed.
