🚨 Attention Hospital Case Management Teams & Discharge Planners 🚨
The TEAMS Model is officially here, and the landscape of post-acute care coordination is evolving.
Since January 26, 2026, this CMS initiative is mandatory, and hospitals across the country are adapting to a new era of collaboration, accountability, and outcomes.
Over the past several weeks, our Administrator and I have been actively visiting local hospitals, meeting with case managers and social workers to provide education on what this means for discharge planning and post-acute partnerships.
To ensure we are ready to support our hospital partners, we have proactively implemented an internal model and designated leadership to oversee this initiative, allowing us to work efficiently, coordinate care effectively, and align with the expectations of this new program.
We are excited and proud to be part of this model, and even more excited to partner with hospitals that are committed to navigating these changes proactively.
