Why Patients Gladly Spend on Everything Except Their Teeth

For decades, dentists have watched the same pattern: patients drive new cars, book vacations, and dine out freely—yet freeze when dental care falls outside insurance coverage.

The problem is historical. Dental insurance was designed in 1954 as a union-funded plan for children's preventive care, not comprehensive adult care. Annual maximums were set when crowns cost under $100. That cap hasn't moved in 60 years, even though adjusted for inflation it should be around $10,000 today.

The solution? Separate diagnosis from cost. Neutralize shame. Use analogies patients understand. Reframe spending as protection, not loss.

When dentists shift the conversation from insurance limits to personal agency, patients begin to see dental health as part of their identity—not just a bill.

The internet loves the line “dentists used to be barbers” and then jumps straight to the punchline: that’s why teeth aren’t covered.

It’s catchy. It’s also wrong.

Dentistry didn’t split from medicine because of a joke or a rejection. It split because medicine didn’t prioritize oral disease, so dentistry built its own schools, licensing, and identity. That helped dentistry professionalize fast, but it also created the silo patients still feel today.

Dental insurance isn’t really insurance. It’s a capped benefit designed for predictability, not protection. That’s why the mouth keeps getting treated like it lives outside the body.

The fix doesn’t start in Washington. It starts in practices. Stop letting coverage dictate diagnosis. Make the mouth–body connection part of everyday care. And explain money honestly, without apologizing for a system you didn’t design.

History isn’t a dunk. It’s a reminder to practice dentistry like oral health is health, even when the benefit booklet says otherwise.
