RN/LPN Per-Diem Home Health Nurses
HHA/CNA Per-Diem

We are searching for RN’s/LPN’s to work Per Diem out in the field who will provide world-class service to our patients based on the foundation of our BELIEFS.

Duties / Responsibilities:

Performs patient initial and ongoing assessments in accordance with MD orders
Initiates a plan of care for the patient, updates as necessary
Provides skilled nursing care within the scope of practice
Documents skilled care in compliance with federal and state regulations as well as Agency policies
Provides patient/family/caregiver education
RN’s will supervise LPN’s and HHA’s
Knowledgeable of federal and state regulations as well as Agency policies and procedures
Communicates with all members of the health care team
Attends Agency meetings and in-service programs as required
Maintains all current/required licensures and certifications
Maintains both Agency and patient confidentiality

Qualifications / Requirements:

Current Florida state license - if required
1-year of experience as RN/LPN; HHA/CNA
Current CPR certification
Health certificate/physical exam and TB testing results
Valid state driver’s license and reliable automobile
Proof of auto insurance
Ability to communicate effectively, both verbally and in writing
Successful completion of background check
Pediatric Specific Requirements for Pediatric Nurse Candidates
HHA/CNA Specific Requirements

Required experience:

1 year of nursing experience required for RN/LPN; 1 year experience preferred for HHA/CNA: 1 year
Required license or certification:
Current RN/LPN License, Current CPR
