Our office will be closed for the Labor Day weekend from 4PM Friday 8/30 - 9AM Tuesday 09/02. We hope you enjoy your extended weekend to wrap up your summer.

*Important Note:* Letters did go out the beginning of August to notify patients that Susan Michalowski, NP, BC will be retiring effective 08/31/2024. We have become aware that some have not received the letters as of yet. We ask that you call immediately to obtain refills in order to transfer care to another provider. We have a short window remaining to assist with this, so call and leave a message if needed, but call NOW! Our staff will do everything we can to assist you with this transition.
