Full Job Description

Accompanies provider into the patient examination room to document the provider's encounter with the patient.
Collaborates with the provider to document the patient history, physical exam, results of diagnostic testing, and diagnosis and disposition.

PRIMARY RESPONSIBILITIES

1. Accompany Physician into the patient examination room in order to transcribe a history and physical exam, and document accurately the physician’s encounter with the patient and others present.
2.List all proper diagnoses and symptoms as well as follow up instructions and prescriptions as dictated by the physician
3. Document all procedures performed by the physician, physician extenders, or nurses, on the medical chart.
4. Transcribe consultations or discussions with family and/or the patient's private physician.
5. Complete the patient's charts by transcribing results of any labs, x-rays, or other evaluations.
6. Continuously check the progress of data availability for orders, to ensure the patient's workup is complete so that the provider is able to make treatment decisions regarding that patient.
7. List all proper diagnoses as well as any discharge/follow-up instructions and prescriptions, under guidance of the physician.
8. May occasionally assist with some patient-related duties.
9. Performs other duties as assigned.

EDUCATION
High school diploma or equivalent

EXPERIENCE
Medical Scribing preferred
Medical Terminology preferred
Basic computer and word-processing skills

Other Requirements:
Ability to multi-task efficiently and effectively.
Must be able to act calmly and effectively in a busy or stressful situation

PHYSICAL REQUIREMENTS
Frequent lifting/carrying and pushing/pulling objects weighing 0-25 lbs.
Frequent sitting, standing, walking, reaching and repetitive foot/leg and hand/arm movements.
Frequent use of vision and depth perception for distances near (20 inches or less) and far (20 feet or more) and to identify and distinguish colors.
Frequent use of hearing and speech to share information through oral communication. Ability to hear alarms, malfunctioning machinery, etc.
Frequent keyboard use/data entry.
Occasional bending, stooping, kneeling, squatting, twisting and gripping.
Occasional lifting/carrying and pushing/pulling objects weighing 25-50 lbs.
Rare climbing.

Job Duties Include:

Arriving a few hours early to the facility to prep for rounds with the MD

Scribe all notes for provider during rounds

Ensure all orders and referrals are given to the facility

Coordinate with the Nurse Practitioner or PA about patients being seen with the provider and giving details of rounds

Help schedule monthly QA/educational meetings in the facility

Due to our model of coordinated care delivery, Spectrum’s team can accommodate interventions which most care facilities are unable to provide.

Our approach allows for the following interventions to be implemented post acute care:

-IV push medications (steroid, diuretics, etc) readily available
-IV antibiotics and fluids, PICC line insertion
-Wound Care
-TPN, PPN, and tube feeding with Dietician and -Speech Therapy
-BiPAP, CPAP, home vent, high flow O2, nebulizers
-STAT lab services

There is a natural step forward beyond requiring full-time IPs. Nursing homes must fully engage their medical directors. The medical director is supposed to be the clinical leader of a nursing home, required under the federal regulations to be responsible for “implementation of resident care policies” and “coordination of medical care in the facility,” a tall order.

Medical directors can’t fulfill that role unless they actually possess the tools and the knowledge base to do so. At Spectrum, each of our providers are practicing Hospitalists working on the front lines each day, and are trained to care for acutely ill patients. While hospitalists have been proven to be one of the best physicians to cover post-acute care, other factors going forward will also make or break SNFs’ ability to succeed under their new reimbursement. For doctors to provide more aggressive treatment—and antibiotics started within four hours—SNF nurses need to be retrained.

Spectrum provides onsite training for SNF nurses when we take on a new facility, and continues to provide monthly in-services on topics requested by facility staff. We also make sure they have the hallmark medications we use in a facility e-kit. According to CMS, 80% of all hospital bouncebacks from SNFs are due to one of five conditions: heart failure, COPD, pneumonia, diabetic complications and UTI, and Spectrum provides training for each of these specific diagnoses.
