It utilizes a team-based care approach providing a personalized, comprehensive healthcare plan to the most at risk, those without access to quality healthcare, the under and uninsured. Those enrolled in our program have better health outcomes, less visits to the emergency room, while continuing to live in their own home often times without ever having to go into a nursing home. What does a day at Valir PACE look like? It looks a lot like support, connection, and peace of mind.

Participants enjoy a mix of medical care, therapy, social activities, and a nutritious meal, all coordinated by one care team that works together to support the whole person. Transportation is provided, and additional in-home support may be coordinated through the program when needed.

It’s a simpler, more connected approach to care designed to help older adults stay independent longer and feel supported every step of the way.

Programs of All-Inclusive Care for the Elderly (PACE) began in the 1970s as a community-based solution to help seniors receive medical care, daily support, and social connection without moving into a nursing home. Today, PACE is a nationally recognized model supported by Medicare and Medicaid.

Programs like Valir PACE bring healthcare, therapy, in-home support, transportation, and social engagement together under one coordinated care team, helping eligible older adults in Central Oklahoma remain safely at home longer.
