It utilizes a team-based care approach providing a personalized, comprehensive healthcare plan to the most at risk, those without access to quality healthcare, the under and uninsured. Those enrolled in our program have better health outcomes, less visits to the emergency room, while continuing to live in their own home often times without ever having to go into a nursing home. We are so proud of two of our wonderful PACE participants, Kendall and Norma, who recently competed in the 2025 Oklahoma Senior Games 1500M Power Walk! They’ve been training with personal trainer Patrick in our fitness center and represented Valir PACE with so much heart and determination.
