We are happy to announce that HPC achieved NCQA (National Committee for Quality Assurance) Patient-Centered Medical Home (PCMH) recognition for its Olathe and Paola sites. This is a significant accomplishment that highlights our commitment to providing high-quality, coordinated and patient-focused care.
According to Amy Falk, CEO, the NCQA PCMH recognition indicates that we have met national standards for patient-centered care, emphasizing care coordination, team-based care and continuous quality improvement. The recognition process involves a rigorous evaluation of HPC's commitment to patient-centered care, access, care coordination and quality improvement processes.
