Reformed Church Home Reformed Church Home is a non-profit Assisted Living, Skilled Nursing, and Sub-Acute Rehab center.

Reformed Church Home has a stellar reputation for quality of care. We have been rated 5-Star Quality by the Centers for Medicare & Medicaid Services since the inception of the rating system, have earned several deficiency-free surveys from the state, and are the only facility in New Jersey to win the QAPI Customer Satisfaction Award this year. We offer a continuum of care with Assisted Living, Sub-Acute Rehab, and Skilled Nursing all in one building. Residents can age in place and couples with different healthcare needs can live under the same roof. We encourage seniors who are at least 65 years of age to consider Reformed Church Home. All faiths are welcomed at Reformed Church Home where we provide a spiritual environment with a full time Reformed Church (Christian religion) Chaplain and visiting religious leaders of several faiths.
