PINK STAFFING GUIDELINE SHEETS

NYS Safe Staffing Guidelines now requires OGH to send copies of any violation to the Staffing Grids for each Unit.
These grids were instituted after an agreement between Management, the Union and ancillary staff.
The violations are reported to the NYS Safe Staffing Committee.
Previously, many copies were not received by the committee due to an unclear process.
If your Unit is NOT staffed per the guidelines, the pink sheet should be filed and given to the NM or MHO. A copy should be made for the Union and placed in the POA envelope.

A POA form should also be filed to protect your license.
Always notify the NM or MHO of your intent to file.
Management should take the PINK and YELLOW forms. The original POA (White) and a copy of the staffing form should be returned to the POA envelope.
If the NM or MHO does not sign forms before the end of your shift, make a notation on the bottom of both forms and leave the unsigned PINK and YELLOW forms on the Unit to be forwarded to the Nursing Office.
Always return the original POA and a copy of the staffing sheet to the envelope.
If you choose to file an ELECTRONIC POA, please fill out the pink staffing form, COPY, and place it in the POA envelope. Ancillary Staff can also fill out a staffing sheet as they are included in the grid, ie: Unit clerk, Aids and techs.

The recent changes in Management on 2nd and 3rd floor were discussed in our Labor Management meeting.

3rd floor—Any PTO (Time off) requests should be sent to
Sara Berlin (in the nursing office)

2nd floor—Any PTO (time off) requests should be sent to
Nicki Lippert

All requests should be submitted on OGH request form and must adhere to the request deadlines posted on every Unit (13 days before schedule is posted.)

A written response should be received within 7 days

NEW CONTRACT HIGHLIGHTS

—3% pay raise effective 1/26/25 Reflected in Pay 2/13/25

—Charge Pay $3.00/hr Effective 1/26/25

—Preceptor pay $3.00/hr Effective 1/26/25

—Increased On-Call pay Effective 1/26/25

—BSN $1,500 upon completion and every May for current Employees and new hires with BSN.

—Certification $800 for full and part time upon hire or completion and every May
$500 for Per Diem

PART TIME Effective 3/23/25

— Available 4 Holidays/yr required

—$3000 one time bonus if hours at or increased to 45 hr/pay period (must increase within next 6 months)

—All requests in writing by posted request date

— Double Pay once scheduled for 45 hours at straight time

PER DIEM Effective 3/23/25

— Available 1 Winter and 1 Summer Holiday

—2 (two) 11.5 hr or 3 (three) 7.5 hr shift per schedule (4 wks)
(2 shifts must be weekend shifts)

—Time MUST be submitted in writing by request date (13 days before schedule posting

—Double Pay after scheduled for 30 hrs straight time per pay period (see Double pay guidelines)

–One time $3000 bonus when bidding on and granted 45 hr Part time position by 2/1/26

DOUBLE PAY Effective 3/23/25

— Double pay per Unit based on 25% vacancy to be Determined by CNO at time of posting

—Vital Links are double time if approved by the CNO

PTO– Paid Time Off Pending completion by HR

—Holiday, Floating Holidays and Vacation hrs will be converted to PTO

—No loss of PTO. May carry up to 95 hours over yearly. Excess will be paid out by 2nd pay period in January of following year.
