Sliding Fee Scale Explained

At Universal Primary Care, your cost of care may be adjusted based on your income.

Our sliding fee program helps ensure that cost is not a barrier to receiving medical care.

It’s confidential.
It’s simple to apply.
And it helps many working families in our community.

What Is an FQHC?

You may have heard that Universal Primary Care is a Federally Qualified Health Center (FQHC).

But what does that actually mean?

It means we are federally supported to provide healthcare services in communities where access may be limited — and no one is turned away due to inability to pay.

We exist to serve you.

📢 A Message to Our Community

The announced closure today of Bradford Regional Medical Center represents a profound change for our community.

For patients and families, this means longer travel times for emergency and inpatient care, increased barriers to specialty services, and added stress during moments when timely access matters most. For seniors, families with young children, and those without reliable transportation, these challenges can feel especially overwhelming.

Hospitals are more than buildings — they are lifelines. They provide stability, employment, and essential services that strengthen the health and economic foundation of rural communities like ours. The loss of these value-added services will be felt across our region.

We also want to recognize and express our deep appreciation for the healthcare professionals, clinicians, support staff, and team members who have dedicated themselves to serving patients at the hospital. Their commitment, compassion, and expertise have made a meaningful difference in the lives of countless individuals and families. Our community is stronger because of their service.

As your community health center, we remain committed to being part of the solution. In the coming weeks, we will be sharing plans for expanded services and the launch of a capital campaign to support a new Bradford facility. This investment reflects our long-term commitment to strengthening access to care locally and ensuring that our community continues to have a strong healthcare foundation.

We will continue providing comprehensive primary care, behavioral health services, preventive care, and care coordination — and we will work closely with regional partners to help patients navigate changes in access to hospital-based services.

This is our home, too. We care deeply about the health and future of our community, and we are committed to moving forward together.

We stand with our community — and we remain here for you. 💙

Our telephone lines are open 24 hours a day, 7 days a week. If the health center is closed, your call will automatically be forwarded to an answering service and they will connect you to our on-call provider.
