Care Coordination Manager (CCM) Emilie Perez recently visited two members at the Hartwick Day Program. The visit included an exciting game of Bingo with member Dan, who nearly won! The Day Program participants had also recently decorated the room with their beautiful Valentine's Day crafts, showcasing their love for the people and things most dear to them. It was a day filled with festive games and crafts—a delight to everyone present!

Southern Tier Connect (STC) CCMs develop personal relationships with each member they support. Emilie’s visit is a great example of the person-centered approach that sets STC care apart!

New York State’s Care Coordination Organizations (CCOs), like STC, provide local care and support to people with intellectual and developmental disabilities (I/DD).

In the upcoming state budget, you can advocate for CCOs to receive Targeted Inflationary Increase (TT, previously known as Cost of Living Adjustment, or COLA). Contact your state legislator through CMANY’s website to ensure that CCOs can maintain a stable workforce, address rising operational costs, and continue providing essential services to 135,000 New Yorkers with I/DD.

Join STC for an interactive panel-presenter workshop designed to demystify the transition process, hosted by the Homer Central School District! There will be a variety of experts from the Office of People with Developmental Disabilities (OPWDD), education, vocational rehabilitation, and community services to discuss what life for students and their families can look like after graduation.

Congratulations to Brittany Riso, Care Manager Supervisor, on their recent graduation!

Last month, Britt graduated from SUNY Binghamton’s Master of Social Work (MSW) program. As a member of the intellectual and developmental disability (I/DD) community, this was a meaningful personal and professional step in her life. For Britt, the passion was always there to provide equitable and informed support; in their words, “I knew I needed the education and training to match my passion.”

This rewarding experience means that Britt feels more prepared to blend her lived experience with formal training to be a strong advocate for the disability community and care coordination managers (CCM) that Britt supports every day.

Norwich and Binghamton offices gifted holiday cheer to children this season!

Both offices ran a Toys for Tots drive, asking their communities to donate toys for children who may not receive many toys this Christmas, and received an incredible outpouring of support!
