To My Wonderful Patients,
My 32 years caring for you in the Mansfield area has been most rewarding and enjoyable. It has been a pleasure serving you and your neighbors in Richland and the surrounding counties. I have enjoyed taking care of your eyes and want to make sure you get the care you need moving forward.
As of September 1, 2024, Ohio Eye Associates, a locally owned practice that has been providing exceptional eye care in the area for many years, will be taking over my practice. I am confident the team at Ohio Eye Associates will be able to continue to care for your eyes with the same dedication and care I have for the past three decades.
I am happy to report that Dr. Michael Finn, OD, who has been practicing with me for the past nine years, will be staying in the same location (1991 Park Avenue West, Ontario, Ohio 44906) and will continue to provide comprehensive eye care at this location.
For questions about your appointment, please call the following numbers:
Scheduled prior to Sep 1, 2024 call (419) 521-3937
Scheduled after Sep 1, 2024 call Ohio Eye Associates at (419) 756-8000
In addition to Dr. Finn, Ohio Eye Associates' team includes two cataract surgeons, seven comprehensive optometrists and a network of specialists providing care for glaucoma, retina and oculoplastics at five area locations.
Dr. Jonathan Skarie, MD, PhD and Dr. Krysta Beshaler, MD are both excellent cataract surgeons practicing at Ohio Eye Associates' location in Mansfield (466 S. Trimble Rd). They offer many options for cataract surgery, including premium lenses such as the "light adjustable lens." Dr. Skarie is also a fellowship trained glaucoma specialist able to provide local care for all stages of glaucoma.
Your medical records will be under the care of Ohio Eye Associates as of September 1, 2024. I recommend Ohio Eye Associates' surgeons, ophthalmologists and optometrists without reservation. I believe they will continue to provide you with the highest level of eye care available.
If you decide to transfer your care to a provider outside of Ohio Eye Associates, to protect the confidentiality of your records, you will need to complete a Request for Medical Records Release document. You may then pick up your records from Ohio Eye Associates or they will be sent to the physician's office of your choice. Please contact Ohio Eye Associates at the above number or your insurance company for any questions about insurance coverage.
It has been a pleasure and honor to be your ophthalmologist. I wish you continued health and wellness.
Kindest Regards,
Harold A. Ballitch II, MD, FACS
