In compounding, the "base" is just as important as the medicine itself! 🧪

We don't use a "one-size-fits-all" approach. We choose a base depending on your specific treatment plan:

Hormones: Specialized bases help estrogen or testosterone carry deeper into the tissue.

Wounds/Infections: Protective ointments stay on the surface to guard the area.

Pain/Inflammation: Deep-penetrating creams target the source of discomfort.

At Accardi Clinical Pharmacy, the base is a vital part of your customized healing.

🎤 Question: "What if I cannot swallow pills? Are there other options?"

If you struggle with traditional tablets, you aren’t alone! One tool we frequently use is an oral disintegrating tablet. Unlike regular pills, you don't swallow these; you simply place them in your mouth and let them dissolve.

Why they work so well:

Convenience: They are usually very small and sugar-based for a quick dissolve.
Efficiency: Much of the medicine is absorbed directly through the lining of the mouth, bypassing the liver to get the full dose into your system faster.

We can often compound a wide range of medications into this form. If you’re ready to ditch the struggle of swallowing large pills, come talk to us! ROGER THAT!

Answer from Dr Roger
Yes, and it makes a huge difference.
We have around twenty five flavors for liquids, including cotton candy and many others.
When we prepare a liquid for a child, we ask the parent and sometimes the child what flavor they prefer.
If a child dreads the taste, they will fight every dose. If we can make it taste a bit more like a treat, everyone’s life gets easier and the child is more likely to get the full treatment.

Caffeine is a central nervous system stimulant.
The amount in one normal cup of coffee, around one hundred twenty milligrams, is not a big deal for most people. It gives a mild lift.
The trouble starts when people stack sources. Morning coffee, then energy drinks that are packed with caffeine, then more later in the day.
You can get too much of anything, including caffeine. That can raise heart rate, increase anxiety, and interfere with sleep.
By the way, tea often has more caffeine than people realize, sometimes more than coffee. So saying I do not drink coffee, I only drink tea does not mean you avoid caffeine.

Answer from Dr Roger

Cream
1 rubs in and goes deeper into the tissue
2 the more you rub, the deeper it reaches

Ointment
1 stays on the surface
2 protects the skin and holds medicine right on top

For example, we often use an antibacterial ointment for a bad cut when we want surface protection.

Gel
1 dries quickly
2 penetrates deeper than an ointment, but usually not as deep as a cream
3 leaves a film of the active ingredient under the skin after it dries

The base we choose depends on what we are treating and where.

I look at history and at the numbers.
One key thing I check is whether their generics are rated AB. That rating means that if you graphed how the drug enters and leaves the body, the generic would match the brand closely.
If the curves match, I feel comfortable. If they do not, I do not want that product on my shelf.
My rule in the pharmacy is simple. If I would not take it myself, we do not buy it.

Answer from Dr Roger

Extended release means the drug is designed to release slowly over time instead of all at once.

There are different designs.
1 a tiny porous bead that looks like a miniature sponge
2 the bead is loaded with a concentrated solution of the drug
3 then it is coated so it does not dump the whole dose at once

As the coating slowly dissolves, fluid gets in, the drug seeps out, and you get a steady release as it travels through your gut.

A delayed release product is a bit different. It is made so that it will not dissolve in the stomach. It waits until it hits the upper small intestine before it starts to release.

It depends on how the pharmacist relates to you.
If the interaction is only "here is your prescription" and a quick hello, then you will not see them as an important player.
A good pharmacist will pull you aside for private conversations when needed, explain what you need to know about a medication so you can use it correctly, and help you feel that their input is just as important as your doctor, nurse, or therapist.
When you understand your medicine and how to use it, your chance of getting the outcome you want goes way up. When you do not, your risk goes up.
You deserve a pharmacist who has time to talk, not only time to count pills.

Answer from Dr Roger
A tiny sip is almost like taking it dry.
You want at least two to three ounces of water with a tablet or capsule. That gives the pill an environment where it can dissolve while it rides the natural movement of your gut.
Think of your digestive tract as an escalator. The pill gets on that escalator. If it has not dissolved by the time it reaches the end, it leaves your body and you only got a fraction of the dose.
If the tablet is five hundred milligrams, you want all five hundred, not one hundred twenty.

Yes. That is the heart of compounding.
With a compounded prescription, I can literally build the medication the way you need it.
I start with the raw ingredients, just like you would gather sugar, flour, and baking soda for a cake. Then I decide on the base and the flavor that works best for your situation.
If you cannot swallow a capsule, we can create a liquid. If a cream works better than a pill, we build a cream.
Compounding is not only for people. We do at least as many prescriptions for pets as we do for humans. Many animal owners are very grateful, because shoving a tablet into a dog is not anyone’s favorite job.
