Position:Case Manager
Location: Hybrid position
Employment Type: Part-Time

We’re looking for a dedicated Case Manager to join our growing team and help us guide patients and families through their healthcare journey with excellence and empathy.
What You’ll Do:
* Coordinate patient care and services across multiple disciplines.
* Develop, monitor, and adjust individualized care plans.
* Conduct assessments and home visits to evaluate needs.
* Advocate for patients and connect them to community resources.
* Ensure timely documentation within established standards.
* Collaborate with providers, nurses, and community partners to achieve quality outcomes.
What We’re Looking For:
* Registered Nurse (RN), Licensed Practical Nurse (LPN), or Social Worker (preferred).
* Experience in case management, care coordination, or community health.
* Strong communication and organizational skills.
* Ability to work independently and as part of a team.
* Compassionate, patient-centered mindset with a drive to make a difference.
