Thankful Thursdays:

“Words cannot express my gratitude for Fyza Laboratory. I work full time, and take care of my 80 year old mother, who still lives at her home. Knowing that my Mother had a UTI, and the thought of taking her to the doctor for tests, would normally be a full day event. Fortunately, I called Fyza, and they brought supplies and retuned the same day to collect her specimen. We had our results the next day, and mom was on to her road of recovery. The opportunity to have a day off of work with my mom, during a time of distress, and have the services she required delivered to her doorstep… was priceless. Molecular diagnostics (MDx) is the rapidly developing area of laboratory medicine that investigates human, viral and microbial genomes and the products they encode. Molecular diagnostic techniques and platforms are playing a larger and more critical role in all areas of anatomic and clinical pathology. In the last decade or so, the clinical laboratory has seen an explosion in the available menu of tests based upon DNA and RNA analysis. The completion of the Human Genome Project and the rapid advancement of technology to arise out of that effort has moved from the research bench to the clinical laboratory bench with swift success. For the first time in the history of the diagnostic laboratory, molecular pathology and diagnostics are extending the range of information available to physicians, pharmacists, geneticists, forensic scientists, research scientists and other healthcare professionals.

While traditional medical and public health laboratory platforms, methodologies and assays are the foundation of laboratory medicine, molecular diagnostics now plays a critical role in patient care.

Traditional urine culture is commonly regarded as the gold standard for detection and identification of pathogens. However, evidence has been accumulating to support use of molecular methods such as PCR.
With antimicrobial resistance becoming more common and complex, effective treatment.

A recent study compared 175 cases of polymicrobial infection, defined as >2 bacteria.
PCR detected 166 and culture detected 39.
Misdiagnosed UTIs, causing 7 million visits to the doctor’s office, 1 million emergency department visits, and over 100,000 hospitalizations annually, with the cost exceeding $2.6 billion each year.
Thus the diagnosis of UTI on the basis of clinical criteria alone has been reported to have an error rate of approximately 33%.

Molecular PCR testing can detect the fastidious organisms that are part of the microbiome; some are clearly pathogens while the role of others is under further investigation.

In this study culture was unable to identify the pathogens in 85% of polymicrobial infections. Such cases may prove to be clinically significant, as mutualism may increase resistance and complicate treatment.

Another advantage of PCR is that since no probe is used to detect lactobacillus, for example, this bacterium, commonly agreed to be a contaminant, is not detected and reported.

UTI symptoms in polymicrobial infections may be caused by a subset of the bacteria, or some combination. Evidence suggests that polymicrobial infections can form cooperative networks that enhance antibiotic resistance, wherein one bacteria species can conder antibiotic renaissance on the other bacterial species.

PCR exhibited greater accuracy than culture for pathogens detected and identified bacteria in 36% of patients who had a negative traditional urine culture.
PCR was much more sensitive in detecting polymicrobial infections than urine culture. The accuracy and speed of PCR testing over traditional urine culture, and the potential identification of polymicrobial infections with the complicated resistance sharing mechanisms is exciting.
