The aftermath of Hurricane Ian has impacted millions of Floridians. The GOCC is committed to help the mutual communities we serve and to always put your patients' health and safety as our priority. In order to best support our shared mission, the GOCC has decided to post-pone the October monthly meeting. The next meeting will be held on Wednesday, November 16th at Encompass Health Rehabilitation Hospital of Altamonte Springs, featuring guest speaker Dr. Larry Hock.

The supports all efforts to improve outcomes and reduce unnecessary hospital readmissions. A significant area of focus is reducing dialysis readmissions and ED visits. Hear a patient's real experiences and learn from an ESRD expert some techniques for improving the transition experience across health care settings and the continuum of care.
