I am writing to inform you that after 20 years of practicing at our Semoran location we will be permanently closed effective October 28, 2022. I would like to thank you for the trust you have given me over the years as your eye care physician. Taking care of you and your family has been a privilege! This location has served us well, and based on our future plans, consolidating into our Lake Nona office is appropriate.

It is important that you continue with medical care. Therefore, all future appointments will be scheduled at our Lake Nona office located at 10006 Wellness Way Suite 100, Orlando FL 32832. If you are unable to see us at our Lake Nona office, we recommend for you to establish care with another physician as soon as possible. All patient records will be kept electronically at our Lake Nona office. Please complete the Medical Release Authorization Form to request your medical records be transferred to your new provider. This form will be located on our website at premiereyeassociates.com under patient forms. Please complete and return to our Lake Nona office.

Our Semoran office will be open on October 20th and 27th from 9am-6pm (closed for lunch from 1pm-2pm). This is only for patients who need to pick up glasses or contacts. Glasses and contact lens orders that have not been picked up or received in our Semoran office will be transferred to the Lake Nona office after October 27, 2022.

Due to impending hurricane Ian we will be closed Wednesday and Thursday. We plan to be open regular hours on Friday, 9-1pm. We hope that everyone stays safe during the storm.
