One of the biggest constraints in value-based care isn’t technology. It’s multi-payer alignment.

As Henish explains, even within Medicare, quality isn’t defined the same way. MSSP and REACH measure success differently, so the same Medicare patient will be managed differently under each program. Add Medicare Advantage to the mix, with its own quality measures, resources, and benefit designs, and complexity increases.

From a clinician’s perspective, this fragmentation makes consistent, patient-centered care harder to deliver. The Centers for Medicare & Medicaid Services ACCESS Model is coming, and it will change how chronic care gets delivered and rewarded. Launching 𝐉𝐮𝐥𝐲 𝟏, 𝟐𝟎𝟐𝟔, the CMS ACCESS Model shifts Medicare from fee-for-service to outcomes, equity, and performance. Success will require coordinated care, measurable improvement, and scalability, supported by strong data, interoperability, and patient engagement.
