⚠️ DAIR failure in acute PJI: what the data says about who fails - and why.
New AAOS 2026 data from 1,000+ PJI cases identified the strongest predictors of DAIR failure:
• >10 days from diagnosis to treatment
• Enterococcus or coagulase-negative Staph present
• Polymicrobial infection
• CRP >100 mg/L

Most actionable: 55.7% failure with empiric therapy vs. 39.5% with pathogen-tailored therapy (P=0.002). IV vs. oral and duration made no significant difference.

"My UTI test came back negative, but I still feel awful."
If you've been there, the problem isn't you. It's the test.

A peer-reviewed study of 44 UTI patients compared traditional urine culture with Next-Generation Sequencing (NGS) in 44 patients with UTI symptoms. Traditional testing missed 70% of real infections, detecting bacteria in only 30% of patients. NGS detected infections in 100% of patients.

Moreover, patients treated based on NGS had significantly better symptom scores than those treated based on culture alone. Culture-negative patients got better when treatment was guided by NGS results.

The hidden issue?

NGS found anaerobic bacteria in 45% of patients, bacteria that the culture tests had missed. In half of those cases, the hidden bacteria were causing the symptoms.

In a 10-year study of 4,000+ surgical PJI cases:
• 40%+ of positive cultures came in after the standard 5-day window (mean: 10 days)
• Common late organisms: C. acnes, coagulase-negative Staph, anaerobes
• Culture-negative failure rate: 60% vs. culture-positive: 31%

Whether the positive came early or late, outcomes were the same- the identification is what counts.
