Closing office location after May 24.

It is with great sadness that I am being forced to vacate our current location. The doctor we were subletting from decided she no longer wanted to share space with us. unfortunately, she has only given us 30 days to leave. In this current market, I have been unable to find adequate space for a new home office despite my best efforts.

Any patient active within the past 4 years wishing to obtain a copy of their records/x-rays, may do so upon request by downloading the Medical Records Release form on our website. We will remain in our current location through May 24th for your healthcare needs. Please call our office @ 407-281-0707 or email your request to info@fogartyorlando.com. We will honor your request as soon as possible. After May 24th, patient may pick up records custodian, Dr. Jared Silberstein of South Park Chiropractic. His office is located at 8865 Commodity Circle. #3 Orlando, FL 32819. Dr. Silberstein's office # is 407-354-0009 to arrange for pick up.

Effective immediately, our office hours on Wednesdays have changed from 9am-noon. We will no longer offer afternoon appointments. We apologize in advance for any inconvenience. We appreciate you and your understanding.
