An Aging Life Care Professional, also known as a Geriatric Care Manager, is an advocate, a guide, and a resource for families caring for an older relative. Assistance is provided to maintain independence, maximize quality of life, and facilitate aging independently and with dignity in the comfort of home. WHAT WE DO
•Identify healthcare and personal needs
•Arrange appropriate in-home assistance
•Act as a liaison between client and family
•Provide medication instruction, education, preparation
•Arrange and accompany client to MD appointments
•Advocate for patient with all healthcare providers
•Organize and manage basic finances
•Coordinate community assistance
•Ongoing adjustments based on client changes and changing needs
•Periodic check-in per client request
•Facilitate smooth navigation of healthcare system
