This podcast episode concentrates on cancer screening, detection, and prevention. Link in bio and story!

We go into detail about the benefits and detriments of screening tests (high sensitivity, low cost), early detection tests (often high cost, high chance incidental finding, but can find cancers that are otherwise not screened for).

Prevention: we discuss modifiable risk factors and which are attributed to the most cancers. Some of these include obesity, to***co, and alcohol. We also discuss smokeless to***co and va**ng.

Prevention: we discuss family history, and hereditary cancer genetic screening (we use Invitae but occasionally dig through full genome sequencing. Polygenic risk scores are not a validated tool for clinical decisions!)

Detection: we discuss screening including from USPTF, ACS (American cancer society), and there differences from other society recommendations.

We also discuss cutting edge tests like ISO-PSA, pirads and Gleason scores, and cancers that cannot be screened for but can be early detected with tests like MRI (including prenuvo and ezra), and the only epigenetic test we order routinely for the right patient (Galleri test, known as Grail liquid biopsy). Yes, we discuss the downsides of these tests as well. The patient needs to know the cons before we order the test.

In my last podcast, I discuss how to optimize your s*xual health. Similar to other areas of health lifestyle is most important. It is reasonable to want optimal s*xual performance, just as it is reasonable to desire, optimal, athletic and cognitive performance.

Like other areas of health, the most powerful interventions are lifestyle changes. They aren’t as s*xy as medications and supplement (pun intended).

Improving exercise is your best first step (120mins zone 2, 2+ days a week of vigorous cardio, and 3+ days a week resistance training). This can lead to improved blood flow, improved core and pelvic muscle strength, and avoid metabolic syndrome and it’s downstream effects.

Diet and supplementation can also be emphasized to contain adequate nitric precursors (for example L-citrulline or other foods with known vasodilator benefit like beet root or garlic).

Stress should be under control in order to balance the “teeter totter/SeeSaw” of fight or flight vs rest and digest. This also includes social and relationship stressors, which are often most important in s*xual health.

Speak with your physician or NP about medications which can help. Balancing dopamine and serotonin is important to avoid premature or delayed or**sm\ejaculation in males and females. APO and even bupropion when indicated can help prevent delayed or**sm and abnormally long refractory periods. PDE5 inhibitors like Tadalafil can be a good addition, especially in older ages where pde5 levels rise naturally. Dose should be the minimally effective dose, usually 2.5mg.

Rancid supplements, optimal form, optimal ratio (different for each individual), Rx vs supplement form, triglyceride vs ethyl ester form, and various benefits.

Have you also noticed an increased incidence of afib anecdotally? What are some of the causes you think contribute? Could high dose EPA without DHA/DPA/ALA be one? Spoiler: the evidence is still mixed but for those at risk please consider your ratio.

Just for fun here is my “DysFunctional med afib stack”: our hypothetical patient is a 75 year old with history of “sluggish thyroid”, “chronic fatigue”, and “adrenal fatigue”. Patient is on daily hydrocortisone “replacement”, armour thyroid 2 grains, modafinil 200mg, pure EPA gems (2g BID because “doesn’t want to take statin”), and was recently given testosterone pellet (unknown dose) and Albuterol inhaler. Patient complains of fluttering in chest and fast heartbeat.

Testosterone levels vary, especially at lower SHBG levels (younger males tend to have lower SHBG). On replacement TRT, even at a level that looks “mid-normal”, the “area under the curve” is greater.

There are exceptions to this, like mostly lymphatically absorbed testosterone “Tlando” and nasal testosterone “Natesto”. Tlando chart is on last slide.

Yes, if your doctor recommends, some patients do use basal subq TRT + bolus morning Tlando or natesto. However, for most there is little upside to doing this.

Most free Testosterone assays are immunoassays, and are not very accurate. You are generally better off calculated your free T (use total T+SHBG).

Why are the same treatments (in traditional medicine) used for s*x offenders and women with PMDD?

Find out in the full podcast where we discuss chemical castration, anti-androgens, SSRI use, and more in the context of PMDD.

PMDD (premenstrual dysphoric disorder) is a common condition that causes a range of both physical and mental symptoms typically in the luteal phase of a woman's menstrual cycle.
A few examples below:

Physical symptoms: fluid retention, breast tenderness, cramping, decreased coordination, body aches
Mental symptoms: anxiety, irritability, depression, food cravings, mood swings

Lifestyle has the capacity to lead to significant improvement as well as significant worsening of symptoms:

For example, alcohol use and poor stress management can certainly exacerbate symptoms in the luteal phase.

The mental symptoms associated with PMDD seem to be tied to progesterone's downstream effect on the GABA receptor via allopregnanolone and seem to depend on an individuals sensitivity to the increase or withdrawal of this hormone.

This can be accomplished in a number of different ways such as stabilizing progesterone or reducing the 5 alpha reduced byproducts of progesterone, allopregnanolone and isoallopregnanolone. (This of these like T3 and reverse T3, the ratio determines the agonist vs antagonist effect).

Fluctuating allopregnanolone levels = symptoms
High allopregnanolone levels with minimal fluctuation = less symptoms
Low allopregnanolone levels with minimal fluctuation = less symptoms
Stable ratio of isoallopregnanolone: allopregnanolone = less symptoms
