TRF—eating within a set window each day—can significantly improve **insulin sensitivity**, reduce **inflammation**, and naturally lower **caloric intake**. It’s not just about when you eat, but how your body responds.

Many of our patients have success with weight loss using GLP-1 agonists like Semglutide and Tirzepatide. However, we stress the importance of weight training. The rising concern is the amount of people who are taking these medications and developing gallbladder disease, significant muscle mass loss, increasing risk of osteoporosis, fractures, pancreatitis, and other health complications. We check labs every 3 months and sooner if needed, frequent check ins to assure our members are hitting the weights.

Here’s a concise and validated overview of the **risks of GLP-1 therapy** and the **importance of strength training while on these medications**:

Risks of GLP-1 Therapy

GLP-1 receptor agonists are widely used for type 2 diabetes and obesity. While they offer significant benefits, including reduced cardiovascular risk and potential neuroprotective effects, they are not without risks:

While whole-body MRIs may sound like a smart way to catch diseases early, they come with risks and limitations:

🔍 False Positives

Can lead to unnecessary anxiety, invasive follow-ups, and even surgeries 1.

1. 💸 High Cost, Low Yield

Often not covered by insurance and rarely improve outcomes in asymptomatic individuals

2. 🧲 Safety Concerns

Not suitable for patients with certain implants or metal in the body

3. 🧪 Lack of Clinical Guidelines

No major medical body currently recommends whole-body MRI for general screening

✅ Smarter, Evidence-Based Alternatives for P*Ps:

1. 🧬 Risk Stratification Tools

Use tools like ASCVD Risk Calculator, FRAX, or QRISK for personalized risk assessment.

2. 🩺 Targeted Screenings

Follow USPSTF guidelines for age- and risk-based screenings (e.g., colonoscopy, mammography, LDCT for smokers). I would recommend Apo B and Lipo (a), homocysteine, B12/folate, Ferritin, B12/Folate, TSH, and a few others

3. 📊 Advanced Lab Panels

Consider lipid subfractionation, HbA1c, hs-CRP, and metabolic panels for deeper insights. In addition, looking at other insulin resistance markers as tools to help assess risk for metabolic disease and prevention of inflammation in the body.

4. 📱 Wearable Tech & Remote Monitoring

Devices like smartwatches can track heart rate variability, sleep, and activity trends.

5. 🧠 Annual Wellness Visits

Comprehensive history, physical exam, and shared decision-making remain gold standard. BUT, with ongoing touch points. The idea that we need 1 visit for a snapshot assessment is misguided. Our "wellness visits" MUST BE ONGOING. Monthly for some, quarterly for others, and at times daily for challenging cases.

We offer primary care at assisted living, independent living, or private home. Our mission is to prevent readmissions after hospitalization or SNF/Rehab discharge. While practicing hospital medicine for more than 10 years, we realize that people need help navigating the health system while treating acute conditions at home now more than ever. As we all age, we need to bring health care to the home. We love having family meetings in the living room and being there to help make difficult decisions. This is what health care is meant to be - together, in pursuit of a quality of life defined individually by each one of us.
