We are seeking a Direct Support Professional to join our team for caring for our amazing residents! You will assist in the daily care of developmentally disabled individuals creating a home environment and delivering compassionate care.

Responsibilities:
Assist clients with daily living activities including meals
Complete programming activities and document outcomes
Communicate ongoing care results and updates to relevant parties
Collaborate with clients and families for best care opportunities
Maintain a healthy and comfortable living environment

Qualifications:

CNA Certification preferred but not required
Previous experience in personal care or other related fields preferred but not required
Compassionate and caring demeanor
Ability to build rapport with clients
Excellent written and verbal communications skills
Ability to work collaboratively and as a team

Shifts Available:

Full time and part time with morning, and afternoon shift. All positions include every other weekend and on call.

Overnight position available- hours 10 p.m. - 6 a.m. which include every other weekend and on call.

Responsibilities:
The Licensed Practical Nurse (LPN) is responsible for providing nursing care to residents, including medications and treatment administration, documentation, and other therapeutic interventions under the direction of the Director of Nursing/Clinical Director. Provides direction to nursing assistants, resident assistants, trained medication assistants, and other clinical staff on unit/shift.

Essential Job Responsibilities:
Provides nursing care to residents within the scope of practice of a Licensed Practical Nurse (LPN). Responsible for coordination and direction of resident care. Coordinates care and services with other departments/providers from the time of admission to discharge. Assists Registered Nurse with completion of assessments, documentation, and data collection within scope of practice and acting timely on findings. Administers medications and completes treatments as ordered. Observes and monitors resident’s condition and reports changes as appropriate. Ensures that all physicians’ orders are signed and implemented within specified time frames. Maintains safety of all residents and employees. Responds appropriately to safety hazards and reports any issues to the appropriate person. Ensures that all staff follows the infection control policy and procedure. Maintains a clean, sanitary, safe and efficiently organized work area.

Job Title: Cook / Dietary Supervisor

Job Summary Description: The cook / dietary supervisor shall prepare and serve all resident meals as directed by the consulting dietician. This individual must be able to cook with a proficient level of skill serving delicious and nutritious meals. Please note that this position cooks for individuals with developmental disabilities and special needs. Family style dining is done at each meal. This individual must be able to be flexible and creative preparing, cooking and cleaning up for all meals while maintaining all department of health and human services safety and cleanliness standards.

Qualifications:
• A minimum of one year of food preparation experience and/or be willing to participate in food preparation classes as designated by the administrator.
• Must be able to pass a background check

Job Responsibilities and Duties:
A. Food Preparation:
a. Follow prescribed menus as outlined by the consulting dietician.
b. Document and record any menu changes and report those changes to appropriate individuals.
c. Responsible for preparing breakfast, lunch and supper meals for the residents and pack lunch for day service facility.
d. Shall assist in planning resident's birthdays and plan holiday menus.
e. Prepare meals that meet the specific needs of the residents.
1. Menu changes to meet diets
ii. Preparing new menus
f. Adhere to sanitary and infection control procedures in the preparation of meals.
B. Food Purchasing:
a. Purchase menu items as needed.
b. Maintain required inventory records.
c. Review with Administrator budgetary guidelines.
d. Ordering Hy-Vee and Sysco
C. Sanitation/Safety:
a. Maintain kitchen equipment and sanitize
b. Clean kitchen in accordance with cleaning schedule.
c. Report malfunction or equipment to Administrator.
d. Follow sanitary guidelines and good personal hygiene practices as directed by consulting dietician and Administrator.
e. Adhere to meal time schedules as required.
D. Attendance at Required Meetings:
a. Attend mandatory staff meetings and training.
b. Participate in annual CPR and First Aid training.
c. Participate in mandatory Vulnerable Adult Training annually.
d. Meet quarterly with Consulting Dietician.
e. Attend quarterly infection control meetings.
f. Attend interdisciplinary team meetings as designated by Administrator.
