Testosterone… in women with a history of breast cancer?
Let’s talk about what the data actually says.
A small 2026 study looked at transdermal testosterone in 47 women with prior breast cancer—and the symptom burden at baseline was high: fatigue (98%), brain fog (96%), low libido (96%).
After ~4 months of treatment:
✨ Significant improvement in overall menopause symptoms
✨ Decreases in anxiety, depression, night sweats, and palpitations
✨ Improvement in 20 out of 24 symptoms measured
And here’s the kicker:
The improvement held even when libido was removed from the analysis → meaning testosterone may be doing more than just affecting s*x drive.
But before we oversimplify this…
⚠️ This was a small, open-label study
⚠️ No placebo group (and menopause symptoms are highly placebo-responsive)
⚠️ No long-term safety data—especially in breast cancer survivors
So what does this actually mean?
👉 Women with a history of breast cancer have very limited options for treating menopause symptoms
👉 Non-hormonal therapies often don’t work or aren’t tolerated
👉 And yet… many clinicians are already using testosterone off-label because patients report feeling better
This study doesn’t give us final answers.
But it does give us something important:
A signal.
A signal that testosterone might help mood, cognition, energy, and quality of life—not just libido.
And a clear message that we need better, larger, randomized trials.
Because women deserve data.
Not dismissal.
