Are you a nurse with a sharp eye for detail, strong communication skills, and a passion for patient follow-up? We’re seeking a compassionate and organized LPN to join our care team.
Job overview:
Responsible for patient care under the direction of a physician or Nurse Practitioner in a clinic setting
Key Responsibilities:
- Patient triage
-Answer and respond to phone call messages and attend to urgent situations as they arise
-Follow standing orders and policy/procedures
-Administer medications/procedures per standing orders/provider orders
-Work prior authorizations
-Document accordingly using our EMR system, etc.
-Scheduling procedures/surgeries
- Review provider’s notes on laboratory, imaging, and test results in the EMR and escalate critical findings per protocol
- Communicate normal and abnormal results to patients clearly and compassionately
- Provide education, instructions, and next steps based on provider recommendations
- Coordinate follow-up appointments, prescriptions, or additional testing as needed
- Ensure documentation is accurate and complete in the patient’s medical record
-Collaborate with physicians, specialists, and care teams to maintain continuity of care
- Follow HIPAA and confidentiality guidelines in all communications
Qualifications:
- Active LPN license
- 1+ year of clinical nursing experience (primary care, specialty, or outpatient preferred)
- Strong clinical judgment and attention to detail
- Excellent communication and patient education skills
- Experience with EMR systems
