I am here to help you! A comprehensive chart review, where we can go over your healthcare records with you. Labs, notes, and more!

This gives you all the information at your pace.

Benefits I have seen from this process with patients:
1.We caught lab abnormalities that were left unaddressed such as a low potassium level that was caused by a medication they were taking.

2. Most patients reach out because something has happened to them they don't full understand. It's typically a surgery, procedure, or hospital admission where they were sent home and don't have all the information or know how to get it. By reviewing notes, labs, and imaging I offer them information to provide these answers so they can ask follow up questions or make decisions about their care.

3. Chronic illness management - People with chronic illnesses have a lot to manage. By reviewing their history from start to finish, we catch all the things that they were not taught, don't remember, or haven't set up a system for success in home. For example, heart failure patients should weigh every day. A weight gain can indicate they are building up fluid which is a common cause of hospital readmissions. Which everyone wants to avoid. If we set up a routine that works for them, make sure they have functioning equipment, follow up for consistency, and early intervention, dramatically decreases their risk of being hospitalized again.

These benefits and more help improve health management and ultimately peace of mind.

It's a simple process....by using either paper copies or electronic records we can go over everything with you at your pace and put together the whole story.

I can guide you on questions to ask and provide you resources to get your needs addressed.

There are two options for your initial review:
1. You can provide the information for a one-hour review process and then do a one hour telephone or virtual review.

Or

2. You can schedule a 2 hour review and we go step by step together.

Whichever option is most convenient for you! Either option is $125. You also gain the ability to text or call with additional questions for up to one week after your initial review. I want to make sure you get all your questions answered and concerns addressed.

"Why did you go independent?"

So I can provide the care you need rather than check boxes for the insurance companies. Your care should be about you. We need a relationship of trust and respect without an administrator or insurance rep in the room.

"How much does it cost?"

Prices range based on complexity. The first 2 hour intake and consultation is comprehensive. This means in just two hours we are going to cover everything and get your most pressing questions addressed. I have never left a patient upset or confused. Everyone books at least one more visit depending on their needs.

"What do you like most?"

I can physically see and feel the relief my patients have after our visits. There's peace of mind in knowledge. That is why I became a nurse. No one should have to stress or be confused by what is happening to them. Everyone should have answers to make decisions that are best for them.
