In order for an individual to be eligible for a qualified patient registry ID card in Kentucky (including in-state qualified patient, and visiting qualified patient applicants), they must have a diagnisis related to one (1) of the following qualifying medical conditions:​

•​Any type or form of cancer;
•Chronic or severe pain;
•Epilepsy or other intractable seizure disorder;
•Multiple sclerosis, muscle spasms, or spasticity;
•Chronic nausea or cyclical vomiting syndrome; or
•Post-traumatic stress disorder.

However, the state of Kentucky DOES REQUIRE I SEE YOU ON VIDEO.
We can do this on FACETIME or ZOOM. If you do not have access to either one of these, you will have to come to my Paducah office for renewal.
Kentucky state RECOMMENDS card holders submit for renewal of their medical card 1-2 MONTHS BEFORE the expiration date to avoid delays, and risk your card expiring!
