Aza Health Rural Health Care, Inc. d/b/a/ Aza Health is a private non-profit community and migrant health center Since its founding in 1977, Rural Health Care, Inc.

d/b/a/ Aza Health is a private non-profit community and migrant health center established in rural Putnam County, Florida in 1977. A federally qualified health center (FQHC), Aza Health operates in areas designated as Health Professional Shortage Areas and or Medically Underserved Areas or Populations. Most Aza Health physicians are Board Certified or board eligible in Family Practice, and Internal Medicine. (Aza Health) has grown to include locations in six contiguous counties in northern Florida: Alachua County, Clay County, Flagler County, Putnam County, St. Johns County, and Volusia County. Today Aza Health operates 11 full-time primary care centers offering a wide range of services, three full-time dental centers, and nine full-service pharmacies. In addition, with more than 250 employees, Aza Health is a major employer in the area. Aza Health receives HHS funding and has Federal Public Health Service (PHS) deemed status with respect to certain health or health-related claims, including medical malpractice claims, for itself and its covered individuals. Aza Health is governed by a volunteer Board of Governors that meets each month. Among their duties: defining and preserving the mission of the organization.

Important Update for Aza Health Patients – Interlachen Center Modified Schedule (Thursday)

Due to continued smoke and visibility concerns in Interlachen, the Interlachen Center will have a modified schedule on Thursday following guidance from the Sheriff’s Office.

• The Interlachen Center will be closed during the morning hours
• Staff will be working from the Palatka Center during this time
• The Interlachen Center will open at 1:00 PM for patients who are unable to travel to Palatka for care

Patients may also choose to receive care at the Palatka location or schedule a telehealth appointment.

Important Update for Aza Health Patients – Interlachen Center Closure

Good morning. Unfortunately, smoke from the nearby wildfire is causing visibility and safety concerns in Interlachen. There have been accidents reported, some roads are closed, and local schools have also been affected.

For the safety of our patients and staff, our Interlachen Center will be closed again today, as staff are unable to safely travel into the area.

All services are being transferred to our Palatka Center, and patients have the following options:

• Visit the Palatka location for in-person care
• Schedule or attend a telehealth appointment

Our team will still be available and working from our Palatka Center, and patients have the following options:
