Here at Gentiva Hospice Palm Desert, we believe everyone can learn how to advocate for themselves and their loved ones by learning about the benefits of hospice. We believe that once you attend one of our orientations, you will want to volunteer to share your skills with our patients and caregivers. Our volunteer orientations are just 1 hour and provide an overview of the hospice benefit. Hours: Mon. - Th./Fri. | 5PM - 8AM (weeknights)
Fri. - Sun./Mon. | 5pm - 8am (weekends)

You will report directly to the Administrator, Executive Director, or Patient Care Manager Senior and will maintain primary control and professional management of each patient. You will act as primary liaison between physician, patient/family, and hospice team.

• You’ll provide direct patient care services to hospice patients and supportive care to the patient’s family after normal business hours on an on-call basis
• You’ll manage an interdisciplinary group consisting of Licensed Practical/Vocational Nurses, Certified Nursing Assistants and support staff to enhance the quality of hospice care
• You’ll evaluate patient/family needs and assist in the preparation and ex*****on of the plan of care for each patient
• You’ll maintain all assigned patient clinical records
• You’ll participate in company quality assessment and performance improvement programs
