Dear Patients,
Due to the extension of the shelter-in-place ordinance, Standard Optometry will be limiting business operations until at least May 31, 2020 to help prevent the spread of COVID-19. Santa Clara County has allowed for routine eye exams as of April 29, so we are carefully planning a slow reopening for the safety of our patients and staff.
Dr. Lo will be seeing limited appointments for the month of May based on urgency. We will be implementing the following office protocols effective immediately for the month of May:
• Appointments for examinations or glasses/contacts must be made in advance as we will be open limited hours and by appointment only so that we can limit the number of people in the office at a time. Pick-up of frames/contact lenses or frame adjustments are also required to have a scheduled time. Please call and leave a message at 650-917-1342 or email us at info@standardopt.com and we will reply as soon as possible.
• Patients and accompanying guardians must wear a face mask for the duration of your time in our office. If you do not have one, we will provide one for you.
• Only the patient is allowed in the office for the appointment, except in the case of minors 18 years and younger. Minors can be accompanied by one legal guardian.
• Patients will be pre-screened before appointments and must confirm they:
- are not currently sick or have an immediate family member who is sick or has flu-like symptoms
- have not traveled in the past 14 days
- have not tested positive for COVID-19 or been in contact with someone who has tested positive for COVID-19
Patients who do not pass the screening questionnaire will be asked to reschedule.
• If you are a vulnerable individual (if any of the following pertains to you), we may ask you to continue to shelter-in-place and reschedule for your safety:
- Persons 65 years and older
- Persons with serious underlying health conditions (i.e. hypertension, chronic lung disease, diabetes, asthma, obesity or if you are immunocompromised)
• New patients to our office will be requested to fill out history forms before the appointment time. We will send a copy via email – please print it out and scan back to us or bring it with you at the time of your visit.
• Please maintain at least a 6 feet distance from other patients and/or staff as much as possible during your visit. We have demarcated places to wait to be helped.
• All equipment, furniture, and shared surfaces will be thoroughly cleaned according to OSHA and CDC recommendations before and between every patient encounter.
