Peninsula Healthcare Connection

Peninsula Healthcare Connection Serving vulnerable populations by providing medical, case-management, and community resources. PHC services the north peninsula of the San Francisco Bay area.

Peninsula HealthCare Connection (PHC) is a not-for-profit organization with a focused mission to serve the needs of the homeless and the at-risk of becoming homeless. Medical, psychiatric and intensive case management is provided within a multi-service building complex with three other entities; Downtown Streets Team, InnVision, and the Housing Authority. The organizations operate cooperatively in partnership to deliver health care, social services and housing. PHC provides services through a collage of community agencies consisting of staff, trainees and volunteers including the Palo Alto Medical Foundation and Stanford University as well as the generosity of many. Our common thread is our mutual commitment to the promotion and maintenance of human dignity, health and safety.

Every single day, you show up prepared to meet people in their toughest moments, bringing your genuine selves, your compassion, and critical resources that serve as a lifeline to our most at-risk community members.

This work carries weight. And the way you carry it, and carry each other, is what makes PHC truly special.

We are proud of everything we have built together and deeply confident in where we are headed. Because of the people at this table, the future of this organization has never been brighter.

Thank you for everything you give to this community.

Our new Memorial Tree stands as a celebration of lives that we have been privileged to know and work alongside of. While they are longer with us, their stories continue to inform the compassion that we bring to our work each day.

Thank you to the El Camino Hospital Social Work team for meeting with us to strengthen how we support patients together. Through New Directions, funded by El Camino Healthcare District, we’re able to help patients with deeper, ongoing support beyond the hospital.

PHC honors Black History Month, a celebration of Black excellence and a reminder that equity work is more important than ever. In Santa Clara County, Black/African American residents are 2% of the population but 12% of people experiencing homelessness. PHC exists to help close gaps like these.

Shoutout to our partners at Silicon Valley Bike Exchange!

Michaela from PHC's New Directions program secured a bike & accessories for a newly housed client. A reliable ride can mean so much for access & stability. We’re deeply grateful for the partnership .exchange

We usually hate graphics with too much text BUT this one had to be done.
Even still, words can’t capture the beauty of this care connection.
In fact, the beloved Ms. Charlotte said the acknowledgement moved her to tears.
Happy Friday, y’all.

It’s not every day we get to pause long enough to be together under one roof. We try to make it a priority, because staying connected to each other helps us best show up for the community we serve.

What you’re witnessing here is a beautiful mix of lived experiences, roles, backgrounds, and perspectives, all held together by our shared mission: to provide comprehensive care and support to the people that need it most.

We’re proud of how much we’ve grown, and even more proud of what we’re protecting as we grow: trust, dignity, and connection.

Thank you for being part of our orbit.
